Support Coordination
Your plan is only useful once it is working
Support coordination is a capacity-building support. We help you understand what is in your plan, turn it into services that actually start, and build the confidence to direct them yourself over time.
We are not here to funnel you into our own services. Your coordinator's job is to find the right fit, whoever provides it, keep everyone aligned, and step in quickly when something is not working.

What your coordinator does for you
You get one point of contact who knows your plan and answers the phone.
- Explaining your plan in plain language
- Finding and comparing providers
- Setting up service agreements and bookings
- Tracking budgets so funding lasts the plan
- Preparing evidence for plan reviews
- Resolving issues and escalating risks
- Coordinating health, housing and community supports
- Building your skills to self-manage
